The Pangea Medical readmission prevention program follows patients both physically and digitally throughout a 30-day period. During this time we are not only acting as a care coordination platform for our patients, but are engaging in activity to help increase both patient self- sufficiency and healthcare literacy.


Our comprehensive in-home assessment integrates both the in-home and clinical data, which not only provides accurate predictions but also helps to identify the most effective means of intervention when required.



The primary focus of the program is to help our partners reduce unnecessary re-hospitalization. Tackling this issue allows our partners to not only reduce costs and avoid heavy penalties, but increase their quality standards, and better focus on the sustainable health of the patient.

Pangea Medical handles the complex and time-intensive coordination process of organizing in-home visits, scheduling, and communication. A job which is often costly to setup and run in-house. We work with our partner’s care management and discharge team in a collaborative manner to not add any additional responsibilities.

Since the introduction of the hospital readmission reduction program, CMS withholds up to 3% of reimbursements to hospitals with high readmission rates. Beginning in 2018, Medicare will also penalize SNFs for above average readmission rates. Reducing readmissions effectively reduce the Medicare penalty. Furthermore, engaging in innovation to better invest in patient outcomes and reduce readmission rates increases the quality ratings of our partners.
